SNP Model of Care (MOC) Exclusions
What is the SNP Model of Care (MOC)?
The SNP Model of Care (MOC) is a framework for Special Needs Plans (SNPs) to meet the unique needs of each enrollee. It is approved by the National Committee for Quality Assurance (NCQA).
Exclusion from the SNP MOC
The SNP MOC includes all of the following except:
- Care coordination
- Medication management
- Behavioral health services
- Long-term services and supports
- Palliative care
- End-of-life care
As provided under section 1859f7 of the Social Security Act, every SNP must have a Model of Care.
